Please be advised that on June 8, 2015, Cook County began a Dependent Eligibility Verification Program conducted by HMS Employer Solutions, an independent third-party audit company. Dependent Eligibility Verification letters are being sent to employee homes with detailed instructions on documentation requirements and deadlines. All employees are required to provide the necessary documents to HMS to verify their dependents. If you have not received your letter from HMS please call 1-844-845-8345 to request a letter or online access instructions.
